By using Reliance Care Solutions's website, intake forms, or care tools, you agree to these Terms. If you do not agree, please do not use the site.

Not medical advice

The content, tools, and reports on this website are for general informational purposes. They are not a substitute for medical advice, diagnosis, or treatment, and they are not an official Medicaid, Ohio Department of Aging, or DODD eligibility determination. Always speak with a qualified clinician, your county case manager, or your SSA for decisions about care, waivers, or benefits.

Care tools and estimates

Our eligibility quiz, cost estimator, care plan builder, and ZIP coverage checker produce estimates based on the information you provide and publicly available program rules. Actual eligibility, costs, hours authorized, and coverage status may differ. We do our best to keep this accurate, but we make no guarantees of completeness or fitness for any specific situation.

No service guarantee

Submitting an inquiry, quiz, or assessment request does not create a provider-patient relationship and does not guarantee that we will accept your case. Active care begins only after you sign a service agreement and we confirm capacity in your area.

Acceptable use

• Do not submit information that is false, harassing, or another person's PHI without authority.
• Do not attempt to disrupt, reverse-engineer, or probe the site for vulnerabilities.
• Do not scrape or republish our content without written permission.

Intellectual property

All content on this site. Copy, logos, graphics, tool logic, PDF templates. Is owned by Reliance Care Solutions or used with permission. You may share links to public pages freely.

Limitation of liability

To the maximum extent permitted by Ohio law, Reliance Care Solutions is not liable for any indirect, incidental, special, or consequential damages arising from your use of the website or the tools. Our total liability for any claim related to the website is limited to $100.

Governing law

These Terms are governed by the laws of the State of Ohio. Disputes will be resolved exclusively in the state or federal courts located in Lucas County, Ohio.
