Reliance coordinators live and work in Toledo, so most assessments are scheduled within four business hours and most starts happen the same week. We cover every Toledo neighborhood, from the historic homes of Old West End to the bungalows of South Toledo, the duplexes of East Toledo, and the ranches of West Toledo and Point Place. We work daily with discharge planners at ProMedica Toledo Hospital, Mercy Health St. Vincent, and Flower Hospital, so we know which floors call early and which ones wait until Friday at 4 p.m.

Quiet, walkable, and a short drive from our Toledo office, Maumee is one of the highest-density PASSPORT pockets we serve. Caregivers cover Wolf Creek, the Side Cut, Indian Hills, and the Conant Street corridor every day. Many of our Maumee clients are longtime homeowners who want to stay in the house where they raised their kids. We work with Mercy Health Perrysburg discharge teams when a Maumee hospitalization routes south.

Sylvania and Sylvania Township stretch from Centennial Quarry north to the Michigan line, and we keep care teams positioned so a Main Street client and a King Road client both get a 20-minute response. We coordinate with Flower Hospital weekly, support memory-care clients in the Olander Park area, and run weekend respite shifts for families balancing work and aging parents. Sylvania is also one of our top markets for live-in and 24-hour private-pay care.

Oregon families get the same Toledo coordinator and the same response window, even though we know the drive across the Veterans Glass City Skyway adds a few minutes. We support clients near Pearson Metropark, along Navarre Avenue, and out toward the refineries where shift-work families need overnight respite. Mercy Health St. Charles is our most common discharge partner here, and we attend the Lucas County PASSPORT staff meetings that include the Oregon caseload.

Holland sits right on the southwest edge of Toledo, and our caregivers cover it from the same coordinator pool. Most families here are working adult children juggling jobs in Springfield Township and aging parents at home. We run a high share of weekend respite and weekday split shifts in Holland, and we coordinate with Springfield Local Schools when a younger DSP client needs after-school support. Toledo Express Airport is minutes away, which matters for our out-of-town family caregivers flying in for ISP meetings.

Waterville is small, historic, and tightly knit, and our caregivers know the difference between the older homes along Farnsworth Road and the new builds out by Pray Boulevard. Many of our Waterville clients are dual-eligible Medicare and Medicaid seniors who want to age in place near the Maumee River. We coordinate with Anthony Wayne Local Schools for younger DSP clients and with Waterville Senior Center on community-based care plans.

Northwood is one of those eastern Lucas County communities people forget about until they need home care, and that is exactly why we built a local caregiver bench here. We support PASSPORT and IO Waiver families near Woodville Road, in the older Walbridge-adjacent neighborhoods, and out toward the I-280 corridor. Northwood Local Schools partners with us on transition planning for graduating DSP-eligible students, and we coordinate with Mercy Health St. Charles in Oregon for hospital-to-home discharges.
